Submit your Integrated Care Best-Practice Project until 5th August!

In June 2013 the SIP Platform has reported about the Compilation of Good Practices for Integrated Care (IC) Projects”, which had been presented during the eHealth week mid-May 2013.

Two German examples for integrated care for chronic pain patients are already included:

  • Second opinion in Back Pain;
  • TK Integrated Care contract for Back Pain;

Now it is your chance to submit your Good Practice Project!!

Please contact the SIP platform to collect more national & regional examples and to ensure that pain will be officially mentioned and recognized as relevant topic within B3 Integrated Care Action Group of the EU Innovation Partnership of Active and Healthy Ageing by the EU Commission.

Here you can find the B3 Good Practice Template.

The good practice examples should be linked to the core action areas of B3 Integrated Care collaborative: change management, risk stratification, organisational models, patient empowerment, ICT/Teleservices, care pathways and finance & funding.
