Compilation of "Good Practice Projects of Integrated Care"
B3 Integrated Care Action Group of the EU Innovation Partnership of Active and Healthy Ageing presented a Compilation of “Good Practice Projects of Integrated Care” at eHealth Week mid-May 2013.
The European Platform for Patients’ Organizations, Science and Industry (Epposi) is the coordinator of the B3 Integrated Care Action Group of European Innovation Partnership of Active and Healthy Ageing (EIP-AHA). The B3 Action Group has recently compiled a “Good Practices for Integrated Care (IC) Projects”, which has been presented at the eHealth week 2013 in Dublin. The good practice examples are linked to the core action areas of B3 Integrated Care collaborative: change management, risk stratification, organisational models, patient empowerment, ICT/Teleservices, care pathways and finance & funding.
Two German examples for integrated care for chronic pain patients are integrated:
- Second opinion in Back Pain;
- TK Integrated Care contract for Back Pain;
With this compilation, there now exists another official European document, where pain is officially mentioned and recognized as relevant topic to ensure active and healthy ageing of the European Population.
Epposi is one of the coordinators of the B3 Integrated Care Action Group of European Innovation Partnership of Active and Healthy Ageing (EIP-AHA), in particular Epposi is responsible for Action area 7 ICT and Teleservices. Last year, Epposi published a White Paper on Chronic Conditions Management where “chronic pain” is included, under its Advanced Innovation Programme on Chronic Conditions Management (AIP-CCM). Furthermore, Epposi has been endorsing SIP for the third year now.
Epposi is an independent, not-for-profit, partnership-based and multi-stakeholder think tank based in Brussels, Belgium. Its goal is to work at the "cutting edge" of European health policy-making, providing members and the wider public with high quality independent research, capacity-building, knowledge exchange and dissemination with the aim of bridging the gap between innovation and improved public health outcomes. Epposi is the only think tank that brings together patients’ organizations, health professionals & academia as well as various healthcare industry companies on an equally-weighted basis.
